Sliding inside bottom of foot

If my foot is sitting properly all the way back in the heel they fit perfectly, but my foot keeps sliding forward, making the toe extend over the front of the shoe and making the shoe look too small. Any ideas on how to fix this? I also worked with a woman who wore open-toed heels constantly and she SWORE by putting baby powder in them before putting them on. Do you know where I could find these? Do you have an A or AA foot ie: skinnier than normal? I have wear an AA which are next to impossible to find in my size.
